Introducing Our New Community Partners - Passport Health Plan by Molina Healthcare!
ChooseWell Communities is blessed with the community partnership of Passport Health Plan by Molina Healthcare, a Kentucky Medicaid insurance provider. Passport will be providing healthcare education, nutrition, and preventative healthcare resources to the families that we serve, as to provide avenues for improved short-term and long-term healthcare outcomes for our parents and children. This past Christmas, Passport visited our virtual programming sessions to read Rudolph the Red-Nosed Reindeer to our families and gifted them with copies of the book, masks, and sack-packs for the holiday season. Parent Highlight: Meet Katie!
Meet Katie, one of the ChooseWell mamas. Katie has been with us since July 2020 and is such a light to those around her. She follows her dreams and is the first one to offer help to any of her ChooseWell sisters who might need a helping hand or someone to talk to.
Katie shares, "I am so grateful for the life that I get to live today. I have an amazing job helping other people and doing God's work. I have custody of my 8-month-old daughter and am actually present in my other children's lives. With the help of ChooseWell Communities, I have a home of my own with beds and rooms for all my children. This Christmas, I was finally able to not only spend Christmas with them for the first time in 4 years, but I also was able to give them tons of gifts. I have support, I have accountability, and above all, I have people who love me and care about me, as well as my children and my recovery. None of this would be possible without ChooseWell."

Also known as the kaleidoscope. That’s what a group of butterflies are officially called. Fun, right?
ChooseWell's kaleidoscope is an active group of philanthropists dedicated to supporting our mission - that every child under five thrives in a home free from parental substance misuse.
Our Butterfly Investors are committed to regular giving, whether monthly, quarterly, or annually, over a period of three years. Not only do these investments provide critical support to our families ($5,000 supports a family through ChooseWell's programming for an entire year!) - they provide us with stable funding, allowing us to solidify strategic partnerships, thus qualifying us for larger, more generous grant funding and allowing us to reach our full potential of serving 70 families at any given time.
